The author presented the samles of desined recording forms with particular emphasis on the important point concentrations and recomended several new provisisions 1) total illness list from birth to present including vaccinations, instead of the
problem
list of Weed's POMR 2) a new establishment of subtile: concurrent diseases (C. D.); this part for a discription of known diseases contineous up to present besides present illnes
3) The discription of the system Review is not neccesary, it can be sufficiently covered by the history of present illness, C. D. & physical examination
4) On the discription of the general condition of the patient at physical examination, it would be very convenient when the usual and important terms printed beforehand on the recording paper to have simple check by the writer.
5) The desined forms for the discharge note, consultation paper, and the recording of emerency care also pressented.
